To purchase a parking decal, a student must have an up-to-date notarized drug and alcohol form on file. Click on the form below to download.  Please bring the completed form to open house when you purchase a decal.  Seniors, you must have this form completed to reserve your spot if you decide to purchase your spot early to paint it.
