Sports Physicals

This is a reminder that PanCare will be at your school on May 28th to perform sports physicals.

As of today, there are no more available appointments.  We have added another opportunity for July 28th.  

Attached you will find the forms that each student must have completed before stepping on the bus to see the provider. Please do not use PanCare Intake forms from previous years, as they have been updated.

As a reminder, please ensure parents/guardians are aware that all forms must be fully completed, and every required parent/guardian signature spot must be signed. The EL2 form includes two sections requiring both the parent and student signatures (Pages 2 & 4).

For students who require an ECG/EKG, please let the parent or guardian know an email address is required.  This is the only way they will receive the completed EL1 after the ECG has been read by a cardiologist.

If a student comes to the bus with any portion of the forms incomplete or missing a required parent/guardian signature, he/she may be asked to reschedule if appointments are still available. Please make sure students have the original completed forms to present to the medical team on the bus.
